Effective July 1, 2016 any youth programs or activities held on University of Utah premises are required to adhere to University Policy 1-015 Safety of Minors. This information is being provided to you as a third-party using University premises. If you have questions about whether your program or activity falls under the policy, please contact Youth Protection and Program Support (YPPS). All requirements must be met prior to holding activities on University premises and can be subject to audit from The Office of Equal Opportunity.
The requirements are listed below.
Under the University Safety of Minors Policy all Covered Programs are required to register with YPPS before the start of the program. This includes third party programs or events operating on University premises.
The registration system is beneficial to both YPPS and the Program. It allows YPPS to be aware of all youth programs, as well as create accurate reporting of data. It allows a program to maintain all their information in one system and track compliance with the policy in one place.

Any Authorized Adults (as defined above under “Registration”) are also required to have a background check. The background check can be done through any vendor of your choice, but must meet standards approved by the University, which shall include utilizing both a national criminal history database and sex offender registry database.
All Authorized Adults are required to receive training on the University Code of Conduct and Reporting Obligations. This training is provided to any registered program and can be done through Canvas. How to access and complete the training in Canvas (Word Document).
A Liability Waiver is required to be signed and kept on file for any minor participating in a covered program. Non University of Utah programs should use the waiver titled “Liability Waiver for Non U of U Event or Activity being held on campus (Word Document)”. These waivers must be collected by the program and turned in to the sponsoring University department for retention. Do not turn them in to YPPS.
